Instagram's save button only bookmarks a Reel inside the app. If you want the actual video file on your iPhone, you can get it in Safari in under a minute.
Public posts onlyNo Instagram login
InstagramFetch runs in your browser, so there's nothing to install from the App Store. You don't sign in to Instagram either. You paste the Reel link and download the file.
One thing to know up front: this works for public Reels only. If the account is private, the download will fail. We don't ask for your login, so we can't reach private posts.
Steps

Open the Reel in the Instagram app.

Tap the paper plane (Share) icon, then tap Copy link.

Open Safari and go to instagramfetch.com, or use the paste box on this page.

Tap the input box, then tap Paste.

Tap Download and wait a few seconds while the Reel loads.

Tap Download MP4 on the result. Safari will ask to download the file. Tap Download.

Open the Files app, go to Downloads, open the video, tap Share, and choose Save Video to move it to Photos.
Tip: If Safari opens the video in a player instead of downloading, press and hold the video, or use the Share button and pick Save to Files.
